Jeff Siegel Scholarship
The Jeff Siegel Memorial Scholarship was created in 2003 for the purpose of remembering Jeff Siegel of G.M.D. Sales. Jeff was a long time corporate member and past industry advisory committee chair of New York State School Food Service Association. Jeff was admired for his professionalism, his commitment to the Association and for his integrity. Jeff never passed a day without telling a story about his children. His love for children inspired his family and the Association to offer a scholarship in his memory.

Special Needs Allergy Form Sample
We use this form, because our school nurses were receiving notes from doctors, chiropractors, nurses, parents, etc. detailing food allergies/intolerances and requesting certain food substitutions. I wanted to have a qualified medical authority signature on a standardized form.

Outstanding Food Service Program Award
The NYSNA Industry Advisory Committee is offering three (3) $500 awards. These awards may be used for registration for state conference including the NYSNA Annual Conference and NYSNA Legislative Conference (LAC). If your district already sends the award winner to these two state conferences the award may be used to attend SNA Annual Conference (ANC) or the SNA Legislative Conference. After the registration fee is paid, the balance of the award will go to offset further expenses such as travel or accommodations. The purpose of the award is for industry to have the opportunity to acknowledge outstanding programs they have seen while doing business with schools and also to recognize and reward Food Service Directors who run an outstanding overall program or have enhanced their program in a specific way to increase participation or improve the healthfulness of their offerings. The three award recipients will be featured in an issue of FOCUS.
